Membership and Forms
LEEP
To get involved in most of LEEP’s activities/events/programs/sports, individuals must become a member.
NEW! In an effort for LEEP to be more affordable and efficient and to increase participation, LEEP memberships will be $20/month that will automatically be deducted from a checking or savings account of participant’s choice (LEEP membership ACH authorization). Please note there will be a maximum of one free ACH transaction per calendar year. A new membership counts as a free ACH transaction. Each additional ACH change will incur a fee of $50. Credit/debit cards, checks and cash will only be accepted for additional activity/event/sport fees.
To become a member, a 2026 LEEP Membership Form also needs to be completed for LEEP will keep on file. This form provides LEEP with personal and medical information for each participant as well as emergency contact information. Participants must have an updated copy of this form on file at LEEP prior to participating in any LEEP activities.
Memberships will include MOST night activities, special events, and dances; however there will still be charges for a limited number of events/activities/sports (e.g. LEEP Elegance and Special Olympics). Registration will still be REQUIRED for ALL activities/dances/events/sports.

In addition, participation in Special Olympics will now be a flat $50 per athlete per calendar year – no matter how many sports the athlete participates in! This fee will be charged when the athlete registers for their first sport of the year.
Scholarships will be available for memberships and activities/events/sports.

Non-members may attend dances for a per dance fee, but registration is REQUIRED. Members can attend dances for free with the exception of LEEP Elegance.
LEEP Special Olympics Teams
LEEP membership is required for participation in Special Olympics. In addition, participation in Special Olympics will now be a flat $50 per athlete per calendar year- no matter how many sports the athlete participates in! This fee will be charged when the athlete registers for their first sport of the year.
- Special Olympics MN Application for Participation Form (updated 8/24) – NEEDS TO BE COMPLETED ANNUALLY AS OF AUGUST 2024! A MEDICAL/PHYSICAL EXAM AND MEDICAL PROVIDER SIGNATURE ARE NO LONGER REQUIRED!
Registration
Once you have all the necessary forms completed and are a current member of LEEP, registering for programs is easy!
- Drop off or mail the membership form (annually) and ACH form to LEEP at 1315 Stadium Rd, Ste 101; Mankato, MN; 56001.
- Forms may also be emailed to info@mankatoleep.org.
- Decide what activities to register for. Registration is REQUIRED for ALL activities/dances/events/sports.
- Call the LEEP office at 507–387-5122 to register (if no one answers, please leave only ONE voicemail). LEEP staff will respond as soon as they can.
- If needed, request a scholarship application when turning in your LEEP membership form and/or before attending or participating in the event or sport. Please call the LEEP office to discuss your individual scholarship needs. If individual needs change, a scholarship application for membership or any event/sport may be submitted and will be considered at any time.
- Debit/credit card payments for additional events/sports are accepted in-person at the LEEP office with a non-refundable convenience fee. Check and cash payments for additional events/sports are also accepted in-person at the LEEP ofice.
